Pre-production form

Performer Contract Student Film This is the employment contract that the producer and each day player must sign for SAG-AFTRA-signatory student theatrical productions. Sample PDF
W-9 Form Request for Taxpayer ID Number and Certification IRS form that is needed when SAG-AFTRA takes a security deposit from the producer and the producer requests that the deposit be placed in an interest bearing account. Fillable Document
