Waiting List: Vaccination Slots 4/8 and 4/10

WEDNESDAY 4 p.m. UPDATE: All slots currently alotted to SAG-AFTRA have been filled. Please add your name to the waitlist noted below. 

The SAG-AFTRA Chicago Local has been offered 50 vaccination appointments through the Chicago Federation of Labor. If you meet the following criteria set by the CFL, please feel free to follow the information below to register.

Criteria/details (PLEASE READ CAREFULLY):

• Must live or work in the city of Chicago
• Must be in category 1A, 1B, 1C
• Must be a SAG-AFTRA Chicago Local member - you will need to provide your membership ID number during registration and on-site
• Vaccination is Johnson & Johnson 
• YOU MUST BE AVAILABLE on the dates and times offered (4/8/2021 at 1340 W. Washington or 4/10/2021 at 2260 S. Grove). PLEASE DO NOT register if you are not available.
• Appointments are first come first serve. 

To register, visit www.chicagolabor.org/vaccine.

• Click on "APPOINTMENTS" and select either Thursday or Saturday
• Click on the orange "SELECT A TIME" button, and select a registration time
• Enter the following access code for the event: EMMY42
• Complete the registration: Enter your name as both the CONTACT INFORMATION and REGISTRATION 1
• Your union/local number is: SAG-AFTRA
• If you do not make it in this round (registration code will be "invalid" when you try to register) and want to be included on a waitlist should appointments become available - please email w/subject line "VACCINATION WAIT LIST" to chicago@sagaftra.org.
